Please complete the enrollment form and submit a $50 enrollment fee per family. After completing the enrollment form, you may pay online or make a check payable to St. Michael Childcare. Your enrollment fee does NOT apply towards your childcare fees.

ATTENDANCE AGREEMENT
The attendance agreement lists the options for attending St. Michael Childcare Center. Once your plan is chosen and you have signed your agreement, you will be responsible to pay the weekly childcare fees associated with the selections you have indicated. If your child is absent one or more days in any given week due to appointment, illness, or other various reasons, your weekly childcare payment will remain the same. Please give the childcare director 2 weeks notice of any attendance agreement changes. Changes to your agreement will require a new attendance agreement form. Parents must complete a “Change of Attendance Agreement” form before your child’s attendance status will be updated. It may take up to two weeks for all changes to be applied.

- SUMMER PROGRAM ATTENDANCE AGREEMENT:
- Please indicate the days in which your child(ren) will be attending St. Michael Childcare Center.
  
  Summer Program requires each child be enrolled for a minimum of 2 days per week.
  
  No Drop-ins are accepted on Field Trip days.

- Enrollment Fee - A $50.00 non refundable enrollment fee per family is due with enrollment form.
  
  Field Trip Fees - The field trip fee is $20.00 per child per trip attended. The center is closed during the time students and staff are away from the center. Field trip information will be posted in advance. Drop-ins are not accepted on field trip days.
  
  Late Pick Up Fees - St. Michael Childcare Center closes at 6:00pm. Pickup between 6:00 pm – 6:15 pm will result in a $25 fee per occurrence. Pickup between 6:16 pm - 6:30 pm will result in a $50 fee per occurrence.
  
  If parents/guardians cannot be contacted and have not arrived by 6:15pm staff will begin calling names listed on the Emergency Card and ask emergency contacts to pick up the child(ren). Repetitive late pick up may result in termination from the program.
